Receipts that open at the right passage

Source links now land on the exact passage instead of the top of a long document.
Receipts were supposed to save you a click, and in long documents they did not: the link opened the right file but left you at the top of page one. Version 1.2 fixes that.
Every receipt now jumps to the passage that supports the sentence and highlights it. We also cleaned up a few cases where a source was listed twice or listed under the wrong title.

Duplicate sources in one answer are merged into a single receipt
Documents renamed after a question was asked still open from older answers
Receipts from Slack threads open the right reply, not the thread root
If a receipt ever lands in the wrong place, use the flag button on it and we will look at it the same day.
